ABSTRACT

Objective@#To explore the associations between sleep duration and negative emotions among junior college students,and to provide reference for mental health promotion among college students.@*Methods@#Cluster sampling method were used to select 2 524 freshmen from a college in Huainan, Anhui Province. Questionnaires were used to investigate general demographic characteristics, sleep timing, negative emotions and other information. The restricted cubic spline and multivariate Logistic regression were used to analyze the relationship between sleep duration and negative emotion among junior college students.@*Results@#The prevalence of depressive symptoms, anxiety symptoms, stress associated symptoms were 19.41%(490), 28.2%(713), 9.9%(250) respectively.The prevalence of negative emotions was higher among boys(24.3%,34.0%,19.1%) than girls(18.7%,27.4%,8.5%). The differences between groups were statistically significant( P <0.01). After adjusting for confounding factors, sleep duration and negative emotions showed a non linear dose response relationship. Compared with the reference group(8-<9 h), sleep duration <7 h was significantly associated with an increased risk of depressive symptoms and stress symptoms, and <8 h was associated with an increased risk of anxiety symptoms. The additional sleep time on weekends ≥5 h was associated with negative emotions compared with the reference group(<1 h)( P <0.01).@*Conclusion@#Short sleep duration and extra weekend sleep are associated with negative emotions. Reasonable sleep schedule among junior college students might be helpful for the prevention and control of negative emotions.

ABSTRACT

Objective@#To investigate the relationship between childhood abuse and recent negative life events with suicidal ideation and attempt, and to provide reference for prevention and control of suicidal behaviors in middle school students.@*Methods@#Stratified cluster sampling method was used to select 7 986 middle school students from Shenzhen, Nanchang and Shenyang as participants. Questionnaire survey was conducted to collect information on demographic indicators, childhood abuse, negative life events, suicidal ideation and suicide attempt, etc. Logistic regression was used to analyze the relationship between childhood abuse experience and negative life events with suicidal ideation and suicide attempt.@*Results@#The prevalence of suicidal ideation and suicide attempt was 22.9% and 4.7%. After adjusted for confounding variables, with the increase of childhood abuse and negative life events, suicidal ideation and attempt in middle school students showed an increasing trend(P<0.05). Compared those with low level of childhood abuse and negative life events, students with high childhood abuse and high negative life events had a higher suicidal ideation and suicide attempt risk, the multiple adjusted odds ratio was 10.43 and 19.75(P<0.05).@*Conclusion@#Childhood abuse and negative life events are important risk factors affecting suicidal ideation and attempt in middle school students. Strategies to reduce childhood abuse and negative life events should be an integral component of targeted mental health interventions.

ABSTRACT

Objective:To evaluate the clinical effect of Qingjin-Huatan Decoction combined with conventional western medicine in the treatment of acute aggravation of bronchiectasis. Methods:A total of 80 patients in Beijing Sixth Hospital from January 2014 to December 2018 were included. Patients were divided into two groups according to random number table method, 40 cases in each group. The control group was treated with conventional western medicine. The research group was treated with Qingjin-Huatan Decoction based on the control group. Both of the two groups were treated continuously for 7 days. Before and after the treatment, their symptoms and signs were scored, forced expiratory volume in one second (FEV1), forced vital capacity (FVC) and peak expiratory flow (PEF) were detected by pulmonary function tester, serum neutrophil elastase (NE) and cathepsin G were detected by ELISA, tumor necrosis factor-α (TNF-α), interleukin-4 (IL-4) and interleukin-6 (IL-6) in sputum were detected by ELISA, and clinical efficacy was evaluated. Results:The total clinical effective rate of the research group and the control group was 90.0% (36/40) and 77.5% (31/40) respectively. The difference between the two groups was statistically significant ( χ2=4.501, P=0.034). After the treatment, the scores of cough, expectoration, phlegm quality, phlegm quantity, fever, shortness of breath, thirst, chest distress and wet rales in the research group were significantly lower than those of the control group ( t values were 15.240, 16.910, 21.761, 18.949, 16.740, 9.285, 11.732, 9.050, 11.193, all Ps<0.001), FEV1, FVC and PEF were all significantly higher than those of the control group ( t values were 5.085, 4.468 and 4.219, respectively, all Ps<0.001), the levels of TNF-α, IL-4 and IL-6 in sputum were all significantly lower than those of control group ( t values were 8.341, 8.884 and 11.352, respectively, all Ps<0.01), and the levels of serum NE (26.20 ± 3.18 μg/L vs. 51.15 ± 5.24 μg/L, t=25.744) and cathepsin G (5.62 ± 1.01 μg/L vs. 8.05 ± 1.33 μg/L, t=9.203) were significantly lower than those of control group ( P<0.001). Conclusions:The Qingjin-Huatan Decoction combined with conventional western medicine can relieve clinical symptoms of patients with acute aggravation of bronchiectasis, improve pulmonary function, inhibit the expression of inflammatory cytokines and proteolysis activities in the airway, and improve the clinical efficacy.

ABSTRACT

Objective To analyze etiology,clinical manifestation and diagnosis process of vertigo and dizziness in children.Method The clinical data of 82 children with vertigo and dizziness treated in our hospital from January 2006 to December 2016 were retrospectively analyzed.Results There were 46 girls and 36 boys with a female:male ration of 1.28:1.The median age of patients was 9.0 years (14 months-18.0 years) and 21 cases were less than 6 years old.The chief complaints were repeatedly falling,crying,walking unsteadily and scratching the ear for unknown reasons.For children aged > 5 years may express " roof or tent rotation";for those aged > 6-< 15 years (n =34),the main complaint was more ambiguous "dizziness";for those aged ≥ 15-≤ 18 years (n =27) with the main complaint of " dizziness" may clearly express the " sense of rotation" or " the feeling of feeling drowsy,the top-heavy sense," and the accompanying deafness,earfullness,tinnitus and so on.Among 82 cases,there were 15 cases of benign paroxysmal vertigo (BPV,18.3%),12 cases of secretory otitis media (SOM,14.6%),11 cases of vestibular migraine (VM,13.4%),9 cases of benign paroxysmal positional vertigo (BPPV,11.0%),8 cases of inner ear malformation (9.8%),8 cases of Meniere's disease (9.8%),7 cases of vestibular neuritis (8.5%),6 cases of sudden deafness with vertigo (7.3%),4 cases of central vertigo (4.9%),1 case of rare genetic disease (1.2%) and 1 case of vertigo due to mental psychological (1.2%).The incidence of BPV,VM and Meniere's disease in girls was higher than that in boys.There was a tendency to self heal in BPV with the age increasing,in 15 BPV cases,8 after 12 years of age,3 after 14 years old had no vertigo attacks,and 4 cases were still in follow-up.The attack frequency of VM was decreased,and the extent was reduced with the age.Children older than 6 years were able to cooperate to vestibular function tests,and the majority completed the tests.Conclusion The analysis shows that the etiology of vertigo and dizziness in children is different from that of adults.Central vertigo and rare genetic disorders with dizziness as the first symptom are of great harmfulness,so intensive observation and multidisciplinary consultations are recommended.

ABSTRACT

Objective:To analyze the etiology of repeatedly attacks of intractable vertigo and some types of sensorineural deafness whose clinical manifestation were not in conformity with the known spectrum diseases,and explore the screening method to prevent missed diagnosis or misdiagnosis, then provide references for clinical diagnosis and treatment for rare etiology. Method:The authors retrospectively analyzed the clinical manifestations, diagnosis, treatment and prognosis from 4 cases of vertigo sufferers and 2 cases of hearing impairment sufferers whose serological tests were positive for syphilis. All these 6 cases were treated with large doses of penicillin aqueous solutions (24 million U/d), multi-times intravenous infusion, the course of the treatment was 14 d. Result:The clinical manifestations of these 6 patients were lack of characteristic, as well as the results of hearing and vestibular function, imaging diagnosis. Positive syphilis detection of serology and cerebrospinal fluid tests were the main diagnostic basis. After anti-syphilis treatment, 5 cases got satisfied clinical symptoms improvement, 1 case suffered from low-tone sensorineural hearing loss, whose hearing fluctuated recurrently. Conclusion:Syphilis infection may damage the Ⅷ cranial nerve and then lead to vertigo and hearing loss, through chronic syphilitic osteitis of temporal bone, atrophy of organ of corti, osteolytic lesion surrounding the endolymphatic duct, and neurosyphilis. For patients presented with intractable vertigo, and those whose clinical manifestations are not in conformity with the known diseases of unilateral ear or bilateral ears rapidly progressive deafness, syphilis serology screening and validation tests are recommended in case of missed diagnosis or misdiagnosis.

ABSTRACT

Objective To analyze the variation tendency of correlative test indexes in the patients with type 2 diabetic mellitus (T2DM) and diabetic nephropathy (DN) .Methods The urinary microalbumin (M‐Alb) ,blood glucose and renal function were de‐termined in 167 cases of T2DM .Then the cases were divided into the simple DM group ,early DN group and clinical DN group ac‐cording to the excretion rates of urinary M‐Alb .The determined results were compared among 3 group and analyzed .Results The age in the simple DM group was smaller than that in the early DN group and clinical DN group (P<0 .05) .The positive rates of u‐rinary M‐Alb and urinary protein were highest in the clinical DN group ,the differences among 3 groups had statistical significance (P<0 .05) .The difference of blood glucose indexes had no statistical difference between the simple DM group and early DN group , the clinical DN group was apparently higher than that in the simple DM group and early DN group (P<0 .05) .The levels of urea (Urea) and creatinine(Cr) in the simple DM group were significantly lower than those in the early DN group and clinical DN group ,the differences among 3 groups had statistical significance (P<0 .01) .Conclusion The severe the DN ,the higher the urina‐ry M‐Alb ,blood glucose and renal function indicators ,the higher the positive rates of urinary protein and urinary glucose ,the more significant the tendency of renal function damage .

ABSTRACT

Objective:To assay aflatoxin B1 in the oil as a pharmaceutical excipient in soft capsules by LC-MS/MS. Methods:Aflatoxin B1 was extracted from the peanut oil in soft capsules by the solvent composed of methanol and 0. 1% formic acid solution, and then centrifuged and the supernatant was purified by neutral alumina cartridges and tested after the concentration with the mobile phase consisting of methanol and 0. 1% formic acid solution with gradient elution at the flow rate of 0. 3 ml·min-1 . 25μl of the tested solu-tion was injected for the analysis at the column temperature of 30℃. Electrospray ionization ( ESI) source was applied and operated in the position ion mode. Multiple reactions monitoring ( MRM) mode was used to quantify the samples. Results:Aflatoxin B1 was in good linearity within the range of 0. 098-1. 960 μg·L-1(r=0. 999 5). The limit of detection was 0. 05 μg·L-1. The average sampling recovery was 97. 73% (n=6) with RSD of 4. 625%. Conclusion:The method is proved to be sensitive, accurate, specified and re-producible, which is referential for the assay of aflatoxin B1 in oily preparations.

ABSTRACT

Using Dachengqi Tang (DCQT) as a model, high performance liquid chromatography (HPLC) fingerprints were applied to optimize machine extracting process with the Box–Behnken experimental design. HPLC fingerprints were carried out to investigate the chemical ingredients of DCQT; synthetic weighing method based on analytic hierarchy process (AHP) and criteria importance through intercriteria correlation (CRITIC) was performed to calculate synthetic scores of fingerprints; using the mark ingredients contents and synthetic scores as indicators, the Box–Behnken design was carried out to optimize the process parameters of machine decocting process under high pressure for DCQT. Results of optimal process showed that the herb materials were soaked for 45 min and extracted with 9 folds volume of water in the decocting machine under the temperature of 140 1C till the pressure arrived at 0.25 MPa;then hot decoction was excreted to soak Dahuang and Mangxiao for 5 min. Finally, obtained solutions were mixed, filtrated and packed. It concluded that HPLC fingerprints combined with the Box–Behnken experimental design could be used to optimize extracting process of traditional Chinese medicine (TCM).

ABSTRACT

OBJECTIVE@#To analyze social-psychological causes of acute exacerbation or re-decompensation of chronic tinnitus and provide theoretical basis for controlling and preventing tinnitus exacerbation and re-decompensation.@*METHOD@#Systemic audiological tests and tinnitus handicap inventory were performed on 136 chronic tinnitus patients with acuteexacerbation or re-decompensation. For the patients with new hearing loss, a further investigation of living conditions and assessment of social support rating scale were utilized. The patients with relatively definite causes were treated accordingly.@*RESULT@#(1) There were 89 patients complained of new changes of hearing, all of whom could tell the definite time point of tinnitus exacerbation, and 5 of them felt the exacerbation of hearing loss meanwhile. (2) Forty-two patients encountered adverse events on life or working, and tinnitus exacerbation occurred within several weeks to 3 months afterwards. Most of these patients could not tell the definite time point of tinnitus exacerbation or re-decompensation. Five cases of tinnitus exacerbation didn't tell any adverse events on life or working, but showed mood disorders, and the anti-anxiety treatment was effective to them. (3) Forty-seven cases without new hearing loss scored significantly lower in SSRS than healthy adults.@*CONCLUSION@#Emerging hearing loss is the main cause of acute exacerbation of chronic tinnitus. To find it in time and give effective treatment can save newly presented hearing loss, cure or relieve tinnitus. Adverse events in life(or working) and short of social support is another important cause of acute exacerbation of chronic tinnitus or decompensation recurrence, which suggests that social-psychological factors besides of hearing loss should be concerned in diagnosis and treatment of tinnitus.

OBJECTIVE@#To investigate whether there is gender difference in the incidence and severity of tinnitus in medical staff (including doctors, nurses, and technicians).@*METHOD@#A total of 354 people (all are medial stuff from hospitals in Guangzhou) were invited to be involved in the investigation and granted a self-reported questionnaire of tinnitus (designed by the authors; based on the scoring method of severity of tinnitus (Liu et al.). Statistical analysis on the data was performed using SPSS Statistics 17.0.@*RESULT@#(1) The incidence of tinnitus of the sample was 43.22%, with that in female higher than in male (P 0.05). (4) There was statistically significant difference among the four sub-items of the questionnaire (P < 0.01), with the mean score of "occurred environment" higher than "the impact on sleeping", "the impact on everyday life", and "the impact on emotion".@*CONCLUSION@#(1) The effort-reward imbalance might be the key factor of the gender difference in the incidence of tinnitus. (2) For patients of tinnitus, improving the knowledge about their symptoms as well as levels of psychological resilience would be helpful to relieve the mental impairment of tinnitus.

OBJECTIVE@#To probe the factors that affect the pure-tone threshold glycerol test and the distribution of frequencies in positive glycerol test.@*METHOD@#We tested the pure-tone threshold before and after glycerol intake in patients with Meniere's disease, and then analyzed the positive rates,distribution of positive frequencies and the time when the maximum threshold changes appeared.@*RESULT@#The positive rate of pure-tone glycerol test was 72.09%, of which the threshold descending rate was 47.67%, and the threshold rebounding rate was 24.42%; the pure-tone threshold changes of descending group and rebounding group were (17.41 +/- 9.92) dB, (13.53 +/- 4.64) dB respectively, and the differences were significant. The distribution of positive frequencies were 250 Hz, 125 Hz, 500 Hz, 1 kHz, 4 kHz, 2 kHz, 8 kHz in descending order. The maximum pure-tone threshold changes mostly appeared within 2 or 3 hours after glycerol intake.@*CONCLUSIONS@#Patient selection, test occasion and the judgement criteria may affect the positive rate of glycerol test; the positive frequencies distribute in the low-frequency region mainly; the maximum pure-tone threshold changes mostly appeared within 2 or 3 hours after glycerol intake.

ABSTRACT

BACKGROUND: Acute renal failure following heart failure assisted circulation have been extensively reported. However, little data have been available concerning morphological analysis of kidney tissues under that condition.OBJECTIVE: To observe morphological change of ischemia/reperfusion kidney in a sheep pulsatile catheter (PUCA) pump short-term support for heart failure model and explore causes of acute renal failure in assisted circulation patients.DESIGN, TIME AND SETTING: Self-contrast animal experiment was performed at the laboratory of Department of Cardiothoracic Surgery, Renji Hospital of Shanghai Second Medical University between July 2003 and April 2004.MATERIALS: PUCA pump was provided by Gerhard Rakhorst, Professor of Biomedical Engineering, University of Groningen.METHODS: After ischemic heart failure in 10 sheep was induced successfully and subsequently ischemia/reperfusion kidney was developed, PUCA pump was activated to support the hemodynamics for 3 hours.MAIN OUTCOME MEASURES: Hemodynamic parameters were monitored and recorded before thoracotomy, heart failure, and every 45 minutes after the support. Kidney biopsy specimens for light and electron microscopy were obtained 3 hours after support.RESULTS: PUCA pump support was successful in 7 of 10 sheep for 3 hours. During support with the PUCA pump,Hemodynamic parameters gradually restored to normal and stable condition, and blood pressure was close to baseline at the end of experiment. On both light and electron microscopy examination, mild acute kidney change was observed after ischemia/reperfusion. Cytosis in renal glomerulus associated with vasodilatation hyperemia, endepidermis in renal tubules hydropic degeneration, vasodilatation hyperemia and Interstitial edema in renal medulla were the main findings.CONCLUSION: PUCA pump could successfully maintain the hemodynamics for 3 hours in a sheep acute heart failure model,but pathological change in ischemia/reperfusion kidney was remained. It is impossible to predict prognosis of renal function on hemodynamic data alone during support.

ABSTRACT

@#Objective To compare the efficacy and safety of mirtazapine and buspirone on panic disorder.Methods 86 cases with panic disorder were divided into two groups: the mirtazapine(30~60 mg/d) group and the buspirone(15~30 mg/d) group.Hamilton Anxiety Scale(HAMA) was adopted to evaluate the efficacy.Safety was evaluated with Treatment Emergent Symptom Scale(TESS),laboratorial and physical examination.Results The effective rate of mirtazapine group and buspirone group was 90.7% and 83.7% respectively 8 weeks after treatment(χ2=1.17,P>0.05).The scores of HAMA of the mirtazapine group decreased more than that of the buspirone group 1 or 2 weeks after treatment(tt=2.94,P<0.01 and t=2.49,P<0.05 respectively).At the end of wk,But there was not significant difference between two groups 4 or 8 weeks after treatment(P>0.05).Some mild side-effects were observed in both groups.Conclusion Mirtazapine shows a similar effect to buspirone and takes effect earlier on panic disorder.

ABSTRACT

OBJECTIVE:To investigate the long-term toxicity of repeated intramuscular injection of Escherichia coli O157∶H7 polysaccharide-conjugated vaccine(O157) in rats so as to provide safety evidence for clinical trials.METHODS:A total of 48 SD rats were randomly assigned to receive either 0.5 mL vaccine(containing 25 ?g polysaccharides) (immunization group,n=24) or phosphate buffered solution (PBS,control group,n=24) with the same volume for 3 times at a dose interval of two weeks.Sacrifice of 6 rats in each group were scheduled at 2 weeks after first immunization,and at 1,3,and 5 weeks after the third immunization,respectively for observation and determination of hematological and biochemical parameters,histopathology,specific antibody,myeloid tissue,the tissues in injection sites,etc.RESULTS:Compared with control group,immunization group showed no significant pathological change except the dynamic regular change of some hematological parameters induced by the immunization,and no immunologic system damage was observed.CONCLUSION:Repeated intramuscular injection with O157 vaccine in rats wouldn't cause overt toxicity and local irritation.

ABSTRACT

Objective To assess the effect of the pulsatile catheter (PUCA) pump support on cardiac resuscitation in sheep,and to provide a new approach for saving cardiac arrest patients. Methods Cardiac arrest was induced by ventricular fibrillation in 11 sheep. These sheep were divided into three groups including no support (n=3), delayed support (n=2) and immediate support (n=6). Time for cardiac resuscitation and the ratio of success to failure in each group were recorded. Hemodynamic parameters including heart rate (HR), cardiac output (CO), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), central venous pressure(CVP),right ventricular end-diastolic pressure (RVEDP),left atrial pressure (LAP), and left ventricular end-diastolic pressure (LVEDP) were monitored and recorded at 5, 60 and 180 min after cardiac resuscitation with PUCA pump. Results Time for cardiac resuscitation in no support group, delayed support group and immediate support group was (38.3±5.8),(43.5±9.2) and (48.7±23.8)minutes, respectively(P>0.05),and the ratio of success to failure was 0/3,0/2 and 5/1, respectively ( P<0.05). After cardiac resuscitation with the PUCA pump support, MAP, SBP and DBP increased gradually(P<0.05).Conclusions PUCA pump can maintain the hemodynamic stability in a sheep model of cardiac arrest,and can thus increase the success rate of cardiac resuscitation. It may be suitable for resuscitating cardiac arrest patients.

1), attributable risk (AR)=13.33%; ⑤Even though the serum specimens were taken from PB patients, the Ab of majouity of patients converted to positive when relapse occurred. In the majority of patients relapsed the levels of IgM-AbL tended to be increasing or pesistently positive. Usually relapse occurred 1-2 years after IgM-AbL was converted to positive. Relapse occurred 11-30 years after the patients were cured. In very rare case downgrading(from tuberculous to borderline leprosy) occurred. ⑥The levels of IgM-AbL gradually decreased in all relapsed patients after effective treatment except one case whose IgM-AbL was persistently positive. Conclusions The above results indicate that the ND-ELISA might be useful in screening early M.leprae infection and in predicting and monitoring the relapse of leprosy, especially in multibacillary patients.

Objective To study the effects of complex polysaccharides immune milk (CPIM) on cellular and humoral immune functions in mice. Method Sixty ICR mice were divided into five experimental groups and a control group. The mice in experimental groups were given i.p. Ganoderma lucidum polysaccharide (GLP) immune milk,Lycium barbarum polysaccharides (LBP) immune milk and different doses (low,medium and high) CPIM,and the control group was given equivalently skimmed and sterilized goat's milk respectively for 15 d.
